The self-service functionality is found on the main menu when you call 0861 235 664. It is easy to use, and the prompts are easy to follow. The options available to you are:

  • Press 1 for self-help service
  • Press 2 for Client Services
  • Press 3 for Direct Sales
  • Press 4 for the Switchboard

Upon pressing 1 for self-service, follow the below steps to access the desired assistance:


Step 1

Follow the voice prompt to select the specific form you need:

  • 1 to request a Beneficiary Nomination Form
  • 2 to request a Death Claim Form
  • 3 to request a Cashback Claim Form
  • 4 to request a Debit Order Form; and
  • 5 to request a Noting of a Cession Form


Step 2

Using our enhanced identification and verification security process, the system will verify your identity and use the contact information we have for you on record.


Step 3 

The system will e-mail your requested form to the e-mail address we have on record for you, or SMS a downloadable link to the cellphone number we have on record for you.

Ensure we always have your correct contact details to receive the forms you request

You can update your contact details in any of the following easy ways:


  1. On the Assupol Client and Rewards Portal:
    • Open the Assupol Client and Rewards Portal on or;
    • On the landing page, login if you are already registered or register as a new user. Keep your ID number handy for this process;
    • Click on ‘Contact details’;
    • Update your contact number, e-mail address, and your physical and postal addresses.
